Physio EP: 39🤔วันนี้จะมาพูดถึงปัญหาที่มาจาก ligament และ scar กัน (Ligament and Scar (motor control dysfunction) related to poor functional movement) 🤔

🛑เมื่อวานมีคนมาปรึกษาด้วยอาการเจ็บรอบข้อเท้าซ้าย กดและเจ็บทั้งด้านในและนอก ให้ยืน single leg stand พบ calcaneus เกิด lateral tilt ร่วมกับมีอาการเจ็บเข่าซ้ายด้านนอก

🔘จากการซักประวัติพบว่าเคยมี sprain Lt. ankle เมื่อหลายปีก่อน และเคยมี injury อื่นๆ หลายตำแหน่ง รวมทั้งคนไข้ชอบเล่นฟุตบอล ตอนยืนเพื่อจะยิงลูกบอลด้วยขาขวาจะรู้สึก postural unstable รวมทั้ง Anterior frontline (AP perturbation: weak) แทบจะไม่ทำงานเลย และขณะทำ standing calf stretch (ขาซ้ายไว้ข้างหลัง) ผู้ป่วยจะรู้สึกเจ็บรอบๆ Lt. ankle

🔘จึงทำการตรวจด้วย NKT testing พบว่า

1️⃣Lt. deltoid ligament neurologically overactive และ Lt. CFL neurologically underactive

2️⃣Lt. MCL neurologically overactive และ Scar ที่อยู่ใต้ head tibia neurologically underactive

Note:

- calcaneofibular ligament (CFL) เกาะจาก calcaneus ไปที่ fibula

- deltoid ligament เป็น ligament ที่หนา และทำหน้าทึ่คอย support ด้านในของ ankle

🔘จากนั้นจึงทำการ Deactivated deltoid ligament และ activated CFL ligament รวมทั้งกระตุ้นการทำงานของ Lt. Psoas เพื่อช่วยในการเพิ่ม proximal stability of core

🔘จากนั้นจึงทำการ Deactivated Lt. MCL of knee และ activated Scar (Fascial distortion in specific direction) รวมทั้งกระตุ้นการทำงานของ Anterior frontline

🔹หลังทำเสร็จพบว่า🔹

➡️single leg stand: more stable

➡️AP perturbation: strong มากๆ

➡️Standing calf stretch: no pain แค่ตึงๆเท่านั้น

♥️Take home message♥️

ปัญหา ligament และ scar เป็นปัญหาที่พบได้บ่อยมากๆ เพราะทั้งคู่ล้วนส่งผลให้ receptor ทำงานผิดปกติไปได้ และเมื่อผิดปกติต่อเนื่อง จะส่งผลให้เกิด movement pattern dysfunction ได้
