🟢Scar หรือแผลเป็น มีหลายประเภทด้วยกัน เช่น🟢

4️⃣Atrophic Scars

5️⃣Stretch Marks

6️⃣Tattoo (ในที่นี้จะถือว่าเป็นอีกประเภทหนึ่งของ scar เพราะมี tissue damage เหมือนกัน)

🟢 ที่นี้มาดูต่อกันว่า Scar เหล่านี้มีผลต่อ motor control ยังไง❓❓ โดยอ้างอิงจาก 3 กลไกพื้นฐานที่สำคัญ

🔸คุณ K Lewit ให้คำอธิบายว่า 'Scars affect soft tissue in all its layers from the skin to the subcutaneous tissues, the superficial and deep fascias, the muscles, and even the tissues of the abdominal cavity.’

🔸คุณ Carolyn Watson ให้คำอธิบายว่า A scar isn't always "a scar." When a scar is formed, afferent nerve fibers are laid down in a greater concentration than normal tissue. This causes the nervous system to potentially react to the extra sensory input a number of different ways. The CNS does not always react, but may react by increasing pain, decreasing function, etc.

🟤อธิบายต่อกันอีกรอบ

🟢 แผลเป็น หรือ Scar เป็นส่วนหนึ่งของ fibrous tissue ที่มาแทนที่ผิวปกติหลังจากบาดเจ็บ Scarsเป็นผลมาจาก biological process ของ wound repair ซึ่งเหมือนกับส่วนอื่นๆของร่างกาย นอกจากนี้Scar ยังส่งผลต่อconnective tissue (fascia)อีกด้วย โดยที่Fasciaทำหน้าที่ห่อหุ้มเนื้อเยื่อต่างของร่างกาย และยังเป็นตัวนำ sensory input เข้าไปที่ระบบประสาทส่วนกลาง (nervous system) ดังนั้นการเกิด fascial Distortion ย่อมส่งผลให้กล้ามเนื้อ หรือtissueต่างๆ เกิด neurologically underactive หรือ overactive ได้ครับ (***แต่ใช่ว่าทุก tattoo หรือ scar จะมีผลต่อทุก movement นะครับ***)

🔹ก่อนหน้านี้ topic เรื่อง scar ที่ต่างประเทศก็ถกเถียงกันพอสมควรครับ(แต่ปัจจุบันเริ่มมีความเข้าใจมากขึ้นว่า scar ในที่นี้หมายถึงผลลัพธ์ที่เกิดขึ้นมาภายใต้ scar คือ fascial distortion นั่นเอง ทำให้ต่างประเทศมีหลายคอร์สที่เกี่ยวข้องกับเรื่องนี้ เช่น McLoughlin Scar Tissue Release เป็นต้น)

เนื่องจาก scar (fascial distortion) เป็นหนึ่งใน top hierarchy of motor control จึงเป็นสิ่งที่สำคัญที่เราต้องให้ความสำคัญ แม้กระทั่ง vaccine scar เมื่อตอนยังเด็กยังส่งผลต่อmovement pattern ได้เลย นอกจากนี้ยังมีหลาย paper ที่พูดถึง scar ว่ามีผลต่อ movement pattern และนี่เป็นเพียงตัวอย่างเท่านั้น เป็นบทความที่น่าสนใจ
